Have a look at the .doc form from INCITS. The required information for each comment is the same as we would use for writing a comment for a paper with an edit. It is just in a format of a lined grid that has to be expanded for each comment.
A much simpler approach would be to have a text version viewable in the USTAG folder on the J3 server that we could all see. At the end, Lorri can transcribe that info into the INCITS form that could be put in the USTAG folder for a final check by those who could read it. Everyone has access to USTAG. Not everyone has access to Google docs.
