Hi folks,

I have just put 09-018r1 on the server. It differs from 09-018 only in
the following ways:

Pages 86, 190, 213 - line number references corrected.

Page 107 - editing instructions clarified.

Page 398 - paragraph number corrected.

Thanks to David Muxworthy and Stan Whitlock for spotting these errors.
