(j3.2006) 007 formatting

LaTeX wants to put CONTENTS and LIST OF TABLES in caps in the footer.
Is this what you want?  Foreword and Introduction are subclauses
(\section, not \chapter) so (trivial) special attention will be needed
to get these into the footers.
